ASQ-3 Materials Kit

Ages and Stages Questionnaire (ASQ-3) is a developmental assessment tool that parents use to test their child's development, offered in partnership with SPICE and the McLean County Health Department. It is for children ages 0-3.

The ASQ-3 Kit is available to check out at the Youth Services Desk. When checked out you'll also receive a questionnaire based on your child's age. Questionnaires and instructions are available in English and Spanish.

Instructions for how to use the kit are included.

Return the kit to the Youth Services Desk, along with the completed questionnaire. Library staff will send the questionnaire to SPICE, and representatives from that agency will contact parents with the child's score and refer families to early intervention services, if necessary.
